Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000057.4(BLM):c.1220G>A (p.Arg407Lys), citing Ambry Variant Classification Scheme 2023: The p.R407K variant (also known as c.1220G>A), located in coding exon 5 of the BLM gene, results from a G to A substitution at nucleotide position 1220. The arginine at codon 407 is replaced by lysine, an amino acid with highly similar properties. However, this change occurs in the last base pair of coding exon 5, which makes it likely to have some effect on normal mRNA splicing. This nucleotide position is highly conserved in available vertebrate species. This amino acid position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site; however, direct evidence is insufficient at this time (Ambry internal data). In addition, as a missense substitution this is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
