Pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_004183.4(BEST1):c.89A>G (p.Lys30Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the BEST1 gene (transcript NM_004183.4) at coding-DNA position 89, where A is replaced by G; at the protein level this means replaces lysine at residue 30 with arginine — a missense variant. Submitter rationale: This sequence change replaces lysine, which is basic and polar, with arginine, which is basic and polar, at codon 30 of the BEST1 protein (p.Lys30Arg). This variant is present in population databases (rs281865218, gnomAD 0.0009%). This missense change has been observed in individuals with autosomal dominant Best vitelliform macular dystrophy (PMID: 10798642, 28559085). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 99769). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt BEST1 protein function with a positive predictive value of 95%. This variant disrupts the p.Lys30 amino acid residue in BEST1. Other variant(s) that disrupt this residue have been observed in individuals with BEST1-related conditions (PMID: 21273940), which suggests that this may be a clinically significant amino acid residue. For these reasons, this variant has been classified as Pathogenic.
