Uncertain significance for Polycystic Kidney disease — the classification assigned by Department of Pathology and Laboratory Medicine, Sinai Health System to NM_001009944.3(PKD1):c.7350G>T (p.Val2450=): The PKD1 p.Val2450= variant was not identified in the literature nor was it identified in the dbSNP, ClinVar, LOVD 3.0, ADPKD Mutation Database or PKD1-LOVD databases. The variant was not identified in the following control databases: the Exome Aggregation Consortium (August 8th 2016) or the Genome Aggregation Database (Feb 27, 2017). This variant was identified by our laboratory in a patient with a co-occurring, pathogenic PKD2 variant (p.802Profs*21), decreasing the likelihood that this variant has clinical significance. The p.Val2450= variant is not expected to have clinical significance because it does not result in a change of amino acid and is not located in a known consensus splice site. The variant occurs at a non-highly conserved nucleotide outside of the splicing consensus sequence, although 2 of 4 in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer) predict a greater than 10% difference in splicing; this is not very predictive of pathogenicity. In summary, based on the above information, the clinical significance of this variant cannot be determined with certainty at this time. This variant is classified as a variant of uncertain significance.
