Likely benign for Polycystic Kidney disease — the classification assigned by Department of Pathology and Laboratory Medicine, Sinai Health System to NM_001009944.3(PKD1):c.6625G>T (p.Val2209Leu): The PKD1 p.Val2209Leu variant was not identified in the literature nor was it identified in the dbSNP, ClinVar, COGR, LOVD 3.0, ADPKD Mutation Database, or PKD1-LOVD databases. The variant was not identified in the following control databases: the 1000 Genomes Project, the NHLBI GO Exome Sequencing Project, the Exome Aggregation Consortium (August 8th 2016), or the Genome Aggregation Database (Feb 27, 2017). The variant was identified by our laboratory in 1 individual with ADPKD, co-occurring with a pathogenic PKD1 variant (c.2914A>T, p.Lys972*), increasing the likelihood that the p.Val2209Leu variant does not have clinical significance. The p.Val2209 residue is conserved in mammals and computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) provide inconsistent predictions regarding the impact to the protein; this information is not very predictive of pathogenicity. In summary, based on the above information the clinical significance of this variant cannot be determined with certainty at this time although we would lean towards a more benign role for this variant. This variant is classified as likely benign.
