Likely pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000350.3(ABCA4):c.6437G>A (p.Gly2146Asp), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces glycine with aspartic acid at codon 2146 of the ABCA4 protein (p.Gly2146Asp). The glycine residue is highly conserved and there is a moderate physicochemical difference between glycine and aspartic acid. This variant is not present in population databases (ExAC no frequency). This missense change has been observed in individual(s) with clinical features of ABCA4-related conditions and/or Stargardt disease (PMID: 11527935, 29854428). In at least one individual the data is consistent with the variant being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 99459). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt ABCA4 protein function. Experimental studies have shown that this missense change affects ABCA4 function (PMID: 12962493). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
