NM_000435.3(NOTCH3):c.1474A>T (p.Ser492Cys) was classified as Uncertain significance by ARUP Laboratories, Molecular Genetics and Genomics, ARUP Laboratories, citing ARUP Molecular Germline Variant Investigation Process: The NOTCH3 c.1474A>T; p.Ser492Cys variant, to our knowledge, is not reported in the medical literature or gene specific databases. This variant is also absent from general population databases (Exome Variant Server, Genome Aggregation Database), indicating it is not a common polymorphism. The serine at codon 492 is highly conserved, and computational analyses (SIFT: tolerated, PolyPhen-2: probably damaging) predict conflicting effects of this variant on protein structure/function. Most pathogenic NOTCH3 variants occur in exons 2-24 and either create or destroy a cysteine residue within an EGF-like domain (Rutten 2014), and thus the p.Ser492Cys variant is consistent with the predominant mechanism of disease in NOTCH3. However, given the lack of clinical and functional data, the significance of the p.Ser492Cys variant is uncertain at this time. References: Rutten JW et al.
