Pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000350.3(ABCA4):c.4195G>A (p.Glu1399Lys), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces glutamic acid, which is acidic and polar, with lysine, which is basic and polar, at codon 1399 of the ABCA4 protein (p.Glu1399Lys). This variant is present in population databases (rs62642573, gnomAD 0.008%). This missense change has been observed in individual(s) with Stargardt disease (PMID: 10958763, 28118664). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 99256). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on ABCA4 protein function. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. For these reasons, this variant has been classified as Pathogenic.
