NM_033360.4(KRAS):c.491G>A (p.Arg164Gln) was classified as Uncertain significance by Women's Health and Genetics/Laboratory Corporation of America, LabCorp, citing LabCorp Variant Classification Summary - May 2015. This variant lies in the KRAS gene (transcript NM_033360.4) at coding-DNA position 491, where G is replaced by A; at the protein level this means replaces arginine at residue 164 with glutamine — a missense variant. Submitter rationale: Variant summary: KRAS c.491G>A (p.Arg164Gln) results in a conservative amino acid change in the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 2e-05 in 251168 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.491G>A in individuals affected with Noonan Syndrome has been reported in the literature. Experimental evidence from Ras activating assays demonstrated the variant to act similarly to wild-type (Smith_2010). No clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014. Based on the evidence outlined above, the variant was classified as uncertain significance until additional evidence of clinical and/or functional importance becomes available.
