Uncertain significance for Severe early-onset pulmonary alveolar proteinosis due to MARS deficiency — the classification assigned by Johns Hopkins Genomics, Johns Hopkins University to NM_004990.4(MARS1):c.2390C>T (p.Thr797Ile), citing ACMG Guidelines, 2015: MARS1 c.2390C>T (rs146851019) is rare (<0.1%) in a large population dataset1 (gnomAD: 44/280448 total alleles; 0.02%; no homozygotes) and has not been reported in ClinVar nor the literature, to our knowledge. Of three bioinformatics tools queried, two predict that the substitution would be tolerated, while one predicts that it would be damaging. The threonine residue at this position is evolutionarily conserved across most mammals. Due to insufficient evidence, we consider the clinical significance of c.2390C>T to be uncertain at this time.
