NM_207585.3(IFNAR2):c.555_559del (p.Ile185fs) was classified as Pathogenic for Primary immunodeficiency with post-measles-mumps-rubella vaccine viral infection by Laboratory of Medical Genetics Unit, Bambino Gesù Children's Hospital, citing ACMG Guidelines, 2015. This variant lies in the IFNAR2 gene (transcript NM_207585.3) at coding-DNA position 555 through coding-DNA position 559, deleting 5 bases; at the protein level this means shifts the reading frame starting at isoleucine residue 185, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: The p.Ile185MetfsTer12 variant in IFNAR2 gene has been identified in a compound heterozygous status with a nonsense variant in a patient with high fever and lethargy after live-attenuated MMR (measles-mumps-rubella) vaccine and was absent from large population studies. Additionally, ex vivo functional studies indicate that the Ile185MetfsTer12 variant causes an impairment of type I IFN-mediated responses (Passarelli, submitted). In summary, the Ile185MetfsTer12 variant meets our criteria to be classified as pathogenic based upon segregation studies, absence from controls and functional evidence.
