Pathogenic for Mevalonic aciduria; Hyperimmunoglobulin D with periodic fever; Porokeratosis 3, disseminated superficial actinic type — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000431.4(MVK):c.976G>A (p.Gly326Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the MVK gene (transcript NM_000431.4) at coding-DNA position 976, where G is replaced by A; at the protein level this means replaces glycine at residue 326 with arginine — a missense variant. Submitter rationale: This sequence change replaces glycine, which is neutral and non-polar, with arginine, which is basic and polar, at codon 326 of the MVK protein (p.Gly326Arg). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with mevalonate kinase deficiency (PMID: 11313769, 18941711, 23979089, 32060250). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 97639). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt MVK protein function with a positive predictive value of 95%. For these reasons, this variant has been classified as Pathogenic.
