NM_001267550.2(TTN):c.21404-3_21404-1delinsA was classified as Pathogenic for Dilated cardiomyopathy 1G; Autosomal recessive limb-girdle muscular dystrophy type 2J by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the TTN gene (transcript NM_001267550.2) at 3 bases into the intron immediately before coding-DNA position 21404 through the canonical splice acceptor site of the intron immediately before coding-DNA position 21404, replacing the reference sequence with A. Submitter rationale: This sequence change affects an acceptor splice site in intron 73 of the TTN gene. It is expected to disrupt RNA splicing and likely results in a truncated or disrupted TTN protein. Information on the frequency of this variant in the gnomAD database is not available, as this variant may be reported differently in the database. Disruption of this splice site has been observed in individual(s) with clinical features of autosomal recessive centronuclear myopathy (Invitae). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. This variant is located in the I band of TTN (PMID: 25589632). Truncating variants in this region have been reported in individuals affected with autosomal recessive centronuclear myopathy (PMID: 23975875, Invitae internal data). Truncating variants in this region have also been identified in individuals affected with autosomal dominant dilated cardiomyopathy and/or cardio-related conditions (PMID: 27869827, 32964742, Invitae internal data). For these reasons, this variant has been classified as Pathogenic.
