Uncertain significance for Congenital sideroblastic anemia-B-cell immunodeficiency-periodic fever-developmental delay syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_182916.3(TRNT1):c.1284_1287dup (p.Tyr430fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the TRNT1 gene (transcript NM_182916.3) at coding-DNA position 1284 through coding-DNA position 1287, duplicating 4 bases; at the protein level this means shifts the reading frame starting at tyrosine residue 430, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This sequence change results in a frameshift in the TRNT1 gene (p.Tyr430Glufs*38). While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 5 amino acids of the TRNT1 protein and extend the protein by an additional 33 amino acids. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with TRNT1-related conditions. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Experimental studies and prediction algorithms are not available or were not evaluated, and the functional significance of this variant is currently unknown.
