NM_004281.4(BAG3):c.30G>A (p.Met10Ile) was classified as Uncertain significance for Abnormality of the musculoskeletal system; Myofibrillar myopathy 6 by Neuberg Centre For Genomic Medicine, NCGM, citing ACMG Guidelines, 2015. This variant lies in the BAG3 gene (transcript NM_004281.4) at coding-DNA position 30, where G is replaced by A; at the protein level this means replaces methionine at residue 10 with isoleucine — a missense variant. Submitter rationale: The missense variant c.30G>A (p.Met10Ile) in the BAG3 gene has not been reported previously as a pathogenic variant nor as a benign variant, to our knowledge. This variant is reported with the allele frequency (0.0004%) in the gnomAD Exomes. This variant has been reported to the ClinVar database as Uncertain Significance. However, no details are available for independent assessment. The amino acid Methionine at position 10 is changed to a Isoleucine changing protein sequence and it might alter its composition and physico- chemical properties. Computational evidence (Polyphen, SIFT and MutationTaster) predicts conflicting evidence on protein structure and function for this variant. The amino acid change p.Met10Ile in BAG3 is predicted as conserved by GERP++ and PhyloP across 100 vertebrates. For these reasons, this variant has been classified as Uncertain Significance.
