Likely benign for Decreased circulating immunoglobulin concentration; Rheumatoid arthritis; Amyoplasia; Recurrent Neisserial infections; Activated PI3K-delta syndrome — the classification assigned by Rarefied Biosciences Lab to NM_005026.5(PIK3CD):c.221A>G (p.Asn74Ser): The PIK3CD c.221A>G (p.Asn74Ser) variant results in a missense substitution of asparagine to serine at codon 74. This residue is not located within a known functional domain of the PIK3CD protein, and the amino acid change is conservative in nature. Functional immunophenotyping showed transitional B cells at 3.6% and T follicular helper (TFH) cells at 12.6%, both of which fall within normal control ranges and do not indicate immune dysregulation. Furthermore, PI3K pathway activity was preserved, as there was no increase in phosphorylated Akt (pAkt) under stimulated or unstimulated conditions, and no evidence of mTOR pathway activation. Computational predictions support a benign classification: REVEL score of 0.268 (Benign Moderate), AlphaMissense score of 0.08109 (Benign Strong), and SIFT score of 0.162 (Benign Moderate). In the absence of functional abnormalities, immune dysregulation, or disease association, and supported by multiple benign computational predictions, the PIK3CD c.221A>G (p.Asn74Ser) variant is best classified as Likely Benign
