Uncertain significance for Juvenile polyposis syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_005359.6(SMAD4):c.20C>G (p.Thr7Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the SMAD4 gene (transcript NM_005359.6) at coding-DNA position 20, where C is replaced by G; at the protein level this means replaces threonine at residue 7 with arginine — a missense variant. Submitter rationale: This sequence change replaces threonine with arginine at codon 7 of the SMAD4 protein (p.Thr7Arg). The threonine residue is highly conserved and there is a moderate physicochemical difference between threonine and arginine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with SMAD4-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C15"). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site, but this prediction has not been confirmed by published transcriptional studies. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
