Uncertain significance for Combined oxidative phosphorylation defect type 27 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_024537.4(CARS2):c.1418A>G (p.Tyr473Cys), citing Invitae Variant Classification Sherloc (09022015): This variant is not present in population databases (gnomAD no frequency). This sequence change replaces tyrosine, which is neutral and polar, with cysteine, which is neutral and slightly polar, at codon 473 of the CARS2 protein (p.Tyr473Cys). This variant has not been reported in the literature in individuals affected with CARS2-related conditions. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function output the following: SIFT: "Tolerated"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0". The cysteine amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function. ClinVar contains an entry for this variant (Variation ID: 957019).
