Uncertain significance for Neuropathy, hereditary sensory and autonomic, type 2A; Generalized epilepsy with febrile seizures plus, type 7 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001365536.1(SCN9A):c.2765G>A (p.Arg922His), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the SCN9A gene (transcript NM_001365536.1) at coding-DNA position 2765, where G is replaced by A; at the protein level this means replaces arginine at residue 922 with histidine — a missense variant. Submitter rationale: This sequence change replaces arginine with histidine at codon 911 of the SCN9A protein (p.Arg911His). The arginine residue is highly conserved and there is a small physicochemical difference between arginine and histidine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with SCN9A-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
