This variant lies in the ACAD8 gene (transcript NM_014384.3) at coding-DNA position 384, where G is replaced by A; at the protein level this means replaces methionine at residue 128 with isoleucine — a missense variant. Submitter rationale: The M128I variant is not observed at a significant frequency in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). This substitution occurs at a position that is conserved across species, and Methionine 128 is located within the binding cavity of the isobutyryl-CoA dehydrogenase (IBD) enzyme (Sass et al. 2004). Furthermore, a different nucleotide substitution (c.384 G>C) also resulting in a M128I missense change has been reported in the homozygous state in a patient with IBD deficiency (Sass et al., 2004).
