Likely benign for Immunodeficiency; Abnormal circulating immunoglobulin concentration; Recurrent infections; Activated PI3K-delta syndrome — the classification assigned by Rarefied Biosciences Lab to NM_005026.5(PIK3CD):c.401C>T (p.Pro134Leu): The PIK3CD c.401C>T (p.Pro134Leu) variant results in a missense substitution of proline to leucine at codon 134. This residue has moderate conservation, with a phyloP100 score of 3.83, and the variant occurs outside of known critical functional domains essential for PIK3CD protein activity. The allele frequency is extremely low at 0.000004095. Functional immune profiling showed T follicular helper (TFH) cells at 10.2%, which is below control levels, and transitional B cells at 14.2%, which is only minimally elevated, suggesting no significant immune dysregulation. Additionally, no aberrant activation of the mTOR signaling pathway was observed, indicating that PI3K signaling remains unaffected. While no publications are available to further support its classification, the absence of functional impact, normal to minimally altered immune parameters, and the lack of mTOR activation collectively support a Likely Benign classification.
