NM_000536.4(RAG2):c.1120G>C (p.Asp374His) was classified as Uncertain significance for Combined immunodeficiency with skin granulomas; Severe combined immunodeficiency, autosomal recessive, T cell-negative, B cell-negative, NK cell-positive by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces aspartic acid with histidine at codon 374 of the RAG2 protein (p.Asp374His). The aspartic acid residue is highly conserved and there is a moderate physicochemical difference between aspartic acid and histidine. This variant is present in population databases (rs772618891, ExAC 0.01%). This variant has not been reported in the literature in individuals affected with RAG2-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
