Uncertain significance for Microcephaly-intellectual disability-sensorineural hearing loss-epilepsy-abnormal muscle tone syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_145207.3(AFG2A):c.2017T>A (p.Ser673Thr), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the AFG2A gene (transcript NM_145207.3) at coding-DNA position 2017, where T is replaced by A; at the protein level this means replaces serine at residue 673 with threonine — a missense variant. Submitter rationale: Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. This sequence change replaces serine with threonine at codon 673 of the SPATA5 protein (p.Ser673Thr). The serine residue is highly conserved and there is a small physicochemical difference between serine and threonine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with SPATA5-related conditions.
