This variant lies in the LDLR gene (transcript NM_000527.5) at 732 bases into the intron immediately before coding-DNA position 191 through coding-DNA position 740, deleting this region. Submitter rationale: This variant is a deletion of the genomic region encompassing exons 3-4 and part of exon 5 (c.191-735_737del) of the LDLR gene. It is expected to disrupt RNA splicing and likely results in an absent or disrupted protein product. This variant has not been reported in the literature in individuals with LDLR-related conditions. Loss-of-function variants in LDLR are known to be pathogenic (PMID: 20809525, 28645073). This variant affects a cysteine residue located within an LDLRA or epidermal-growth-factor (EGF)-like domains of the LDLR protein. Cysteine residues in these domains have been shown to be involved in the formation of disulfide bridges, which are critical for protein structure and stability (PMID: 7548065, 7603991, 7979249). In addition, missense substitutions within the LDLRA and EGF-like domains affecting cysteine residues are overrepresented among patients with hypercholesterolemia (PMID: 18325082). For these reasons, this variant has been classified as Pathogenic.
