Uncertain significance for Cardiovascular phenotype — the classification assigned by Ambry Genetics to NM_001267550.2(TTN):c.63794-1G>A, citing Ambry Variant Classification Scheme 2023. This variant lies in the TTN gene (transcript NM_001267550.2) at the canonical splice acceptor site of the intron immediately before coding-DNA position 63794, where G is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.36599-1G>A intronic variant results from a G to A substitution one nucleotide upstream from coding exon 134 of the TTN gene. This exon is located in the A-band region of the N2-B isoform of the titin protein and is constitutively expressed in TTN transcripts (percent spliced in or PSI 100%). This variant was reported in individual(s) with features consistent with dilated cardiomyopathy (DCM) (Hazebroek MR et al. Circ Heart Fail, 2018 Mar;11:e004682). Note, this variant is also referred to as c.63794-1G>A (NM_001267550.1) in the literature. This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice acceptor site and will result in the creation or strengthening of a novel splice acceptor site. This variant disrupts the canonical splice site and is expected to cause aberrant splicing. However, although direct evidence is unavailable, this alteration is predicted to result in an in-frame transcript that is not expected to trigger nonsense-mediated mRNA decay.
