Likely pathogenic for Catecholaminergic polymorphic ventricular tachycardia 1 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001035.3(RYR2):c.677G>A (p.Gly226Glu), citing Invitae Variant Classification Sherloc (09022015): In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has been observed in individual(s) with clinical features of catecholaminergic polymorphic ventricular tachycardia (Invitae). In at least one individual the variant was observed to be de novo. This variant is not present in population databases (ExAC no frequency). This sequence change replaces glycine with glutamic acid at codon 226 of the RYR2 protein (p.Gly226Glu). The glycine residue is highly conserved and there is a moderate physicochemical difference between glycine and glutamic acid.
