Uncertain significance for Combined oxidative phosphorylation deficiency; Charcot-Marie-Tooth Neuropathy X — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_004208.4(AIFM1):c.370C>A (p.Pro124Thr), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the AIFM1 gene (transcript NM_004208.4) at coding-DNA position 370, where C is replaced by A; at the protein level this means replaces proline at residue 124 with threonine — a missense variant. Submitter rationale: This sequence change replaces proline, which is neutral and non-polar, with threonine, which is neutral and polar, at codon 124 of the AIFM1 protein (p.Pro124Thr). This variant is present in population databases (rs751325295, gnomAD 0.005%), including at least one homozygous and/or hemizygous individual. This variant has not been reported in the literature in individuals affected with AIFM1-related conditions. ClinVar contains an entry for this variant (Variation ID: 947693). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt AIFM1 protein function with a negative predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
