Uncertain significance for Muscular dystrophy-dystroglycanopathy (congenital with brain and eye anomalies), type A14; Muscular dystrophy-dystroglycanopathy (congenital with intellectual disability), type B14; Autosomal recessive limb-girdle muscular dystrophy type 2T — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_021971.4(GMPPB):c.388_389delinsAG (p.Glu130Arg), citing Invitae Variant Classification Sherloc (09022015): This variant is not present in population databases (ExAC no frequency). This sequence change replaces glutamic acid with arginine at codon 130 of the GMPPB protein (p.Glu130Arg). The glutamic acid residue is highly conserved and there is a small physicochemical difference between glutamic acid and arginine. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Not Available"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Not Available"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site, but this prediction has not been confirmed by published transcriptional studies.
