NM_005026.5(PIK3CD):c.5C>T (p.Pro2Leu) was classified as Likely pathogenic for Recurrent pneumonia; Meningitis; Activated PI3K-delta syndrome by Rarefied Biosciences Lab: The PIK3CD c.5C>T (p.Pro2Leu) variant results in a missense substitution at amino acid 2, replacing a highly conserved proline residue with leucine. This variant is very rare in population databases (3 of 1,614,088 alleles in gnomAD v4). Structural studies do not directly shed light on this region, though this region of the protein lies close to an interface with the p85 inhibitor. Immune profiling revealed elevated transitional B cells at 12% and increased T follicular helper (CD4+ CD45RO+ CXCR5+ PD1+) cells at 22%, both exceeding proportions observed in healthy controls. Additionally, activation of the mTOR signaling pathway was observed in circulating B cells, further supporting the pathogenic mechanism, as mTOR is a critical downstream effector of PI3K signaling. These findings show the same pattern of immune dysregulation as seen in those with pathogenic gain-of-function PIK3CD variants. The clinical phenotype associated with this variant also aligns with the gain of function disorder. Functional studies and rareness of the variant support a Likely Pathogenic classification.
