Uncertain significance for Cardiovascular phenotype — the classification assigned by Ambry Genetics to NM_002471.4(MYH6):c.4750G>A (p.Glu1584Lys), citing Ambry Variant Classification Scheme 2023. This variant lies in the MYH6 gene (transcript NM_002471.4) at coding-DNA position 4750, where G is replaced by A; at the protein level this means replaces glutamic acid at residue 1584 with lysine — a missense variant. Submitter rationale: The p.E1584K variant (also known as c.4750G>A), located in coding exon 31 of the MYH6 gene, results from a G to A substitution at nucleotide position 4750. The glutamic acid at codon 1584 is replaced by lysine, an amino acid with similar properties. This alteration has been reported in a hypoplastic left heart syndrome cohort; however, clinical details were limited (Tomita-Mitchell A et al. Physiol Genomics, 2016 Dec;48:912-921). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
