This variant lies in the SCN1A gene (transcript NM_001165963.4) at coding-DNA position 716, where C is replaced by G; at the protein level this means replaces alanine at residue 239 with glycine — a missense variant. Submitter rationale: For these reasons, this variant has been classified as Pathogenic. This variant disrupts the p.Ala239 amino acid residue in SCN1A. Other variant(s) that disrupt this residue have been observed in individuals with SCN1A-related conditions (PMID: 19303743, 17347258, 28202706), which suggests that this may be a clinically significant amino acid residue. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has been observed in an individual with clinical features of epileptic encephalopathy (Invitae). In at least one individual the variant was observed to be de novo. This variant is not present in population databases (ExAC no frequency). This sequence change replaces alanine with glycine at codon 239 of the SCN1A protein (p.Ala239Gly). The alanine residue is highly conserved and there is a small physicochemical difference between alanine and glycine.
