Uncertain significance for Developmental and epileptic encephalopathy, 27; Intellectual disability, autosomal dominant 6 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000834.5(GRIN2B):c.3400_3416del (p.Leu1134fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the GRIN2B gene (transcript NM_000834.5) at coding-DNA position 3400 through coding-DNA position 3416, deleting 17 bases; at the protein level this means shifts the reading frame starting at leucine residue 1134, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This sequence change creates a premature translational stop signal (p.Leu1134Lysfs*21) in the GRIN2B gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 351 amino acid(s) of the GRIN2B protein. This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with GRIN2B-related conditions. ClinVar contains an entry for this variant (Variation ID: 940412). Experimental studies and prediction algorithms are not available or were not evaluated, and the functional significance of this variant is currently unknown. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
