NM_001369.3(DNAH5):c.13540T>C (p.Cys4514Arg) was classified as Uncertain significance for Primary ciliary dyskinesia by Ambry Genetics, citing Ambry Variant Classification Scheme 2023: The p.C4514R variant (also known as c.13540T>C), located in coding exon 78 of the DNAH5 gene, results from a T to C substitution at nucleotide position 13540. The cysteine at codon 4514 is replaced by arginine, an amino acid with highly dissimilar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. This amino acid position is highly conserved in available vertebrate species, except fish. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this variant remains unclear.
