Uncertain significance for Long QT syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000719.7(CACNA1C):c.5636T>G (p.Ile1879Ser), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the CACNA1C gene (transcript NM_000719.7) at coding-DNA position 5636, where T is replaced by G; at the protein level this means replaces isoleucine at residue 1879 with serine — a missense variant. Submitter rationale: This sequence change replaces isoleucine with serine at codon 1879 of the CACNA1C protein (p.Ile1879Ser). The isoleucine residue is weakly conserved and there is a large physicochemical difference between isoleucine and serine. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has not been reported in the literature in individuals with CACNA1C-related conditions. This variant is not present in population databases (ExAC no frequency).
