Uncertain significance for Brugada syndrome 8 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_005477.3(HCN4):c.2571C>G (p.Phe857Leu), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces phenylalanine with leucine at codon 857 of the HCN4 protein (p.Phe857Leu). The phenylalanine residue is highly conserved and there is a small physicochemical difference between phenylalanine and leucine. This variant is present in population databases (rs756140673, ExAC 0.002%). This variant has not been reported in the literature in individuals affected with HCN4-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function output the following: SIFT: "Tolerated"; PolyPhen-2: "Not Available"; Align-GVGD: "Class C0". The leucine amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
