Uncertain significance for Maple syrup urine disease — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000709.4(BCKDHA):c.359A>G (p.Tyr120Cys), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces tyrosine with cysteine at codon 120 of the BCKDHA protein (p.Tyr120Cys). The tyrosine residue is highly conserved and there is a large physicochemical difference between tyrosine and cysteine. This variant is present in population databases (rs764284154, ExAC 0.003%). This variant has not been reported in the literature in individuals affected with BCKDHA-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
