Pathogenic for Mucopolysaccharidosis, MPS-IV-A — the classification assigned by 3billion to NM_000512.5(GALNS):c.463G>A (p.Gly155Arg), citing ACMG Guidelines, 2015. This variant lies in the GALNS gene (transcript NM_000512.5) at coding-DNA position 463, where G is replaced by A; at the protein level this means replaces glycine at residue 155 with arginine — a missense variant. Submitter rationale: The variant is observed at an extremely low frequency in the gnomAD v4.1.0 dataset (total allele frequency: 0.002%). Predicted Consequence/Location: Missense variant In silico tool predictions suggest damaging effect of the variant on gene or gene product [REVEL: 0.98 (>=0.6, sensitivity 0.68 and specificity 0.92); 3Cnet: 0.99 (>=0.6, sensitivity 0.72 and precision 0.9)]. The same nucleotide change resulting in the same amino acid change has been previously reported as pathogenic/likely pathogenic with strong evidence (ClinVar ID: VCV000093178 /PMID: 9298823). The variant has been observed in at least two similarly affected unrelated individuals (PMID: 24120057, 32024277). A different missense change at the same codon (p.Gly155Glu) has been reported to be associated with GALNS related disorder (PMID: 15235041). Therefore, this variant is classified as Pathogenic according to the recommendation of ACMG/AMP guideline.
