Likely pathogenic for Mucopolysaccharidosis, MPS-IV-A — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000512.5(GALNS):c.1558T>C (p.Trp520Arg), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces tryptophan, which is neutral and slightly polar, with arginine, which is basic and polar, at codon 520 of the GALNS protein (p.Trp520Arg). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with mucopolysaccharidosis type IVA (PMID: 34387910, 34472180). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 93170). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt GALNS protein function. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
