Uncertain significance for Lethal congenital glycogen storage disease of heart — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_016203.4(PRKAG2):c.1070C>T (p.Thr357Ile), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces threonine, which is neutral and polar, with isoleucine, which is neutral and non-polar, at codon 357 of the PRKAG2 protein (p.Thr357Ile). This variant is present in population databases (no rsID available, gnomAD 0.0009%). This variant has not been reported in the literature in individuals affected with PRKAG2-related conditions. ClinVar contains an entry for this variant (Variation ID: 925872). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt PRKAG2 protein function with a negative predictive value of 95%. This variant disrupts the p.Thr357 amino acid residue in PRKAG2. Other variant(s) that disrupt this residue have been determined to be pathogenic (Invitae). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
