Pathogenic for Citrullinemia type I — the classification assigned by Women's Health and Genetics/Laboratory Corporation of America, LabCorp to NM_054012.4(ASS1):c.793C>T (p.Arg265Cys), citing LabCorp Variant Classification Summary - May 2015: Variant summary: ASS1 c.793C>T (p.Arg265Cys) results in a non-conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be disruptive. The variant allele was found at a frequency of 8e-06 in 251342 control chromosomes. c.793C>T has been observed in individuals affected with Citrullinemia type I (e.g. Gupta_2012, Kimani_2015, Kilic_2017). These data indicate that the variant is likely to be associated with disease. A different variant affecting the same codon has been classified as likely pathogenic/pathogenic by our lab (c.794G>A, p.Arg265His), supporting the critical relevance of codon 265 to ASS1 protein function. Additionally, at least one publication reports experimental evidence evaluating an impact of p.Arg265Cys on protein function. The most pronounced variant effect resulted in <2% of normal activity (Berning_2008). The following publications have been ascertained in the context of this evaluation (PMID: 18473344, 22885444, 30035404, 25179242). ClinVar contains an entry for this variant (Variation ID: 92373). Based on the evidence outlined above, the variant was classified as pathogenic.
