Uncertain Significance for Cardiac arrhythmia — the classification assigned by All of Us Research Program, National Institutes of Health to NM_000335.5(SCN5A):c.5764C>T (p.His1922Tyr), citing ACMG Guidelines, 2015. This variant lies in the SCN5A gene (transcript NM_000335.5) at coding-DNA position 5764, where C is replaced by T; at the protein level this means replaces histidine at residue 1922 with tyrosine — a missense variant. Submitter rationale: This missense variant replaces histidine with tyrosine at codon 1923 of the SCN5A protein. Computational prediction is inconclusive regarding the impact of this variant on protein structure and function (internally defined REVEL score threshold 0.5 < inconclusive < 0.7, PMID: 27666373). This variant is found within a highly conserved C-terminus region (a.a. 1772-2016). Rare non-truncating variants in this region have been shown to be significantly overrepresented in individuals with Brugada syndrome and long QT syndrome (PMID: 32893267). To our knowledge, functional studies have not been reported for this variant. This variant has not been reported in individuals affected with SCN5A-related disorders in the literature. This variant has been identified in 1/249240 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
