Uncertain significance for Succinate-semialdehyde dehydrogenase deficiency — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001080.3(ALDH5A1):c.518G>A (p.Arg173His), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces arginine, which is basic and polar, with histidine, which is basic and polar, at codon 173 of the ALDH5A1 protein (p.Arg173His). This variant is present in population databases (rs369366567, gnomAD 0.1%). This variant has not been reported in the literature in individuals affected with ALDH5A1-related conditions. ClinVar contains an entry for this variant (Variation ID: 907464). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on ALDH5A1 protein function. This variant disrupts the p.Arg173 amino acid residue in ALDH5A1. Other variant(s) that disrupt this residue have been observed in individuals with ALDH5A1-related conditions (PMID: 34234304, 37962671), which suggests that this may be a clinically significant amino acid residue. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
