This variant lies in the TF gene (transcript NM_001063.4) at coding-DNA position 1027, where C is replaced by T; at the protein level this means replaces arginine at residue 343 with tryptophan — a missense variant. Submitter rationale: Variant summary: TF c.1027C>T (p.Arg343Trp) results in a non-conservative amino acid change located in the Transferrin-like domain (IPR001156) of the encoded protein sequence. Three of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 0.001 in 1614156 control chromosomes, predominantly at a frequency of 0.0013 within the Non-Finnish European subpopulation in the gnomAD database, including 2 homozygotes. The observed variant frequency within Non-Finnish European control individuals in the gnomAD database is higher than the estimated maximal expected allele frequency for a pathogenic variant in TF causing Atransferrinemia phenotype (0.0011), strongly suggesting that the variant is a benign polymorphism found primarily in populations of Non-Finnish European origin. To our knowledge, no occurrence of c.1027C>T in individuals affected with Atransferrinemia and no experimental evidence demonstrating its impact on protein function have been reported. Two submitters have cited clinical-significance assessments for this variant to ClinVar after 2014. All submitters classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as likely benign.
