This variant lies in the KCNJ2 gene (transcript NM_000891.3) at coding-DNA position 224, where C is replaced by G; at the protein level this means replaces threonine at residue 75 with arginine — a missense variant. Submitter rationale: This sequence change replaces threonine, which is neutral and polar, with arginine, which is basic and polar, at codon 75 of the KCNJ2 protein (p.Thr75Arg). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with Andersen-Tawil syndrome (PMID: 12796536, 16571646). In at least one individual the variant was observed to be de novo. It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 8928). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt KCNJ2 protein function with a positive predictive value of 95%. Experimental studies have shown that this missense change affects KCNJ2 function (PMID: 16571646). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. This variant disrupts the p.Thr75 amino acid residue in KCNJ2. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 15911703, 16217063, 17341397, 17582433, 18452873, 24861851). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic.
