Likely pathogenic for Ataxia-telangiectasia-like disorder — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_005591.4(MRE11):c.1442C>A (p.Thr481Lys), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces threonine, which is neutral and polar, with lysine, which is basic and polar, at codon 481 of the MRE11 protein (p.Thr481Lys). This variant is present in population databases (rs137852762, gnomAD 0.003%). This missense change has been observed in individual(s) with ataxia-telangiectasia-like disorder (PMID: 15269180, 33624863, 33956305). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 8785). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Experimental studies have shown that this missense change affects MRE11 function (PMID: 15269180). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
