Uncertain significance for Glycogen storage disease, type V — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_005609.4(PYGM):c.1748A>G (p.His583Arg), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces histidine with arginine at codon 583 of the PYGM protein (p.His583Arg). The histidine residue is highly conserved and there is a small physicochemical difference between histidine and arginine. This variant is present in population databases (rs760543538, gnomAD 0.003%). This variant has not been reported in the literature in individuals affected with PYGM-related conditions. ClinVar contains an entry for this variant (Variation ID: 877374). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt PYGM protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
