Pathogenic for Early-infantile DEE — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001330260.2(SCN8A):c.4436T>C (p.Ile1479Thr), citing Invitae Variant Classification Sherloc (09022015): ClinVar contains an entry for this variant (Variation ID: 869447). This sequence change replaces isoleucine, which is neutral and non-polar, with threonine, which is neutral and polar, at codon 1479 of the SCN8A protein (p.Ile1479Thr). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of developmental and epileptic encephalopathy (Invitae). In at least one individual the variant was observed to be de novo. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C0"). This variant disrupts the p.Ile1479 amino acid residue in SCN8A. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 25568300). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic.
