Uncertain significance for Leber congenital amaurosis 2; Retinitis pigmentosa 20 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000329.3(RPE65):c.1150G>T (p.Val384Phe), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the RPE65 gene (transcript NM_000329.3) at coding-DNA position 1150, where G is replaced by T; at the protein level this means replaces valine at residue 384 with phenylalanine — a missense variant. Submitter rationale: This sequence change replaces valine, which is neutral and non-polar, with phenylalanine, which is neutral and non-polar, at codon 384 of the RPE65 protein (p.Val384Phe). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with RPE65-related conditions (Invitae). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 866183). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt RPE65 protein function with a positive predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
