Uncertain significance for Diffuse cerebral and cerebellar atrophy - intractable seizures - progressive microcephaly syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_005051.3(QARS1):c.2153C>T (p.Ala718Val), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the QARS1 gene (transcript NM_005051.3) at coding-DNA position 2153, where C is replaced by T; at the protein level this means replaces alanine at residue 718 with valine — a missense variant. Submitter rationale: This sequence change replaces alanine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 718 of the QARS protein (p.Ala718Val). This variant is present in population databases (no rsID available, gnomAD 0.03%), including at least one homozygous and/or hemizygous individual. This variant has not been reported in the literature in individuals affected with QARS-related conditions. ClinVar contains an entry for this variant (Variation ID: 857563). Algorithms developed to predict the effect of missense changes on protein structure and function output the following: SIFT: "Tolerated"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0". The valine amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
