Uncertain significance for Muscular dystrophy-dystroglycanopathy (congenital with brain and eye anomalies), type A14; Autosomal recessive limb-girdle muscular dystrophy type 2T; Muscular dystrophy-dystroglycanopathy (congenital with intellectual disability), type B14 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_021971.4(GMPPB):c.433G>A (p.Gly145Ser), citing Invitae Variant Classification Sherloc (09022015): In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has not been reported in the literature in individuals with GMPPB-related conditions. This variant is present in population databases (rs771732077, ExAC 0.002%). This sequence change replaces glycine with serine at codon 145 of the GMPPB protein (p.Gly145Ser). The glycine residue is highly conserved and there is a small physicochemical difference between glycine and serine.
