NM_001927.4(DES):c.359C>A (p.Ala120Asp) was classified as Likely pathogenic for Desmin-related myofibrillar myopathy by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces alanine, which is neutral and non-polar, with aspartic acid, which is acidic and polar, at codon 120 of the DES protein (p.Ala120Asp). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with arrhythmogenic right ventricular compaction, dilated cardiomyopathy and sudden cardiac death (PMID: 24200904). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 854289). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt DES protein function with a positive predictive value of 95%. Experimental studies have shown that this missense change affects DES function (PMID: 24200904, 31371504). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
